Transitional Housing Placement Program (THPP)
Who it's for  Youths in foster care
Ages 16 and17
GoalsLearn independent living skills in preparation for subsequent transitional housing such as THP Plus Foster Care and THP Plus Aftercare.
HousingLive with roommates in apartments and single-family dwellings 
Requirements and supervisionRegular support and supervision provided by THPP provider staff, county social workers, and ILP case managers.
Transitional Housing Placement Non-minor Dependent (THP -NMD)
Who it's for  Young adults participating in extended foster care. 
Ages 18 to 21  
Housing 

Options include:

  • Single site: a home or complex owned by provider with onsite staff or monitor. 
  • Scattered site: an apartment or room rental in the community. 
  • Host family model: live with a previous permanent connection.
Requirements and supervisionMeet criteria of Extended Foster Care 
Meet regularly with case manager to ensure you are progressing towards your goals.
Transitional Housing Placement Plus (THP Plus or THP Plus-AC)
Who it's for  
  • Emancipated youths
  • Ages 18 to 25  
Housing 

Options include:

  • Scattered site: an apartment or room rental in the community.
  • Host family model: live with a previous permanent connection.
  • Single site: an apartment that allows for shared housing specifically for foster youths.
Requirements and supervision
  • Receive services through THP Plus-AC for three years or until your 25th birthday.
  • Work closely with your case managers to ensure you are progressing towards your goals identified in the Step TILP (Supportive Transitional Emancipation Program Transitional Independent Living Plan). 
  • You must be employed or attend school.
  • Maintain regular contact with your case manager.
Housing Navigator and Maintenance Program (HNMP)
Who it's for  
  • Homeless or at-risk of homelessness 
  • Current and former foster youths
  • Ages 18 to 24
GoalsConnect homeless and/or at-risk of homeless, current and former foster youth to housing programs, including the FYI/HCV program, by developing a housing plan, and assistance to identify housing option.
