Get a ride to your dine-in meal site

Mobility Management Program (MMP)

This program offers free transportation services to qualified participants of the Senior Nutrition Program so they can reach their meal sites and get together with others.

Transportation benefitWho qualifies?
Monthly bus passesProgram participants aged 65 or older who regularly dine at a SNP meal site.
Gas cards Program participants aged 60 or older who regularly dine at a SNP meal site.
Paratransit rides 60 or older with disabilities and unable to use bus and light rail transit service due to physical, visual, or cognitive disabilities. To use paratransit rides, you must be cleared by the Santa Clara Valley Transportation Authority.

Ready to apply?

If you qualify for MMP and want to apply, complete the Senior Nutrition Mobility Management application and bring it to the dine-in site manager for approval.
